What we do and why we're different.
Our Health Advocates will work with clients and/or their families to help them achieve healthier lives and increased well-being. Among the activities they might perform are:
- Helping clients prepare for their medical appointments
- Accompanying clients to medical appointments
- Helping clients learn about managing chronic conditions
- Helping clients prepare for upcoming hospital stays and assisting with the transition back to their homes
- Assistance with developing a personal plan for medication management
- Putting a plan of care together
- Screening, arranging, and monitoring in home help
- Acting as a liaison to families at a distance
- Helping clients and their families come to agreement on important health issues and decisions
- Helping clients find legal assistance, if needed
- Helping clients make appropriate Medicare decisions
- Helping eligible clients understand and work through the Medicaid Managed Care Long-Term Care process
- Providing caregiver support and education
- Well-being checks and regular updates by phone or by skype
- Conducting TCARE* Assessments


*A TCARE assessment (Tailored Caregiver Assessment and Referral) is the only family caregiver burnout prevention solution that is evidence-based, and that is accredited by the Administration for Community Living and approved by the Centers for Medicare & Medicaid Services (section 1115).
